Lighting Controls Field Service Request

This Field Service Request serves as the official agreement between Litesource and the requesting Electrical Contractor listed herein.
What Type of Service Do You Need?(Required)
The item below applies to any level of Field Service including Return Trips

OWNER TRAINING: Once Field Service dates have been scheduled, the Electrical Contractor shall coordinate a day and time with the end-user (Owner) and the assigned Lighting Controls Technician for Owner Training. If owner training is unable to be completed within the scheduled startup date(s), a Billable Return Trip will be required. Refer to Acknowledgement.

BASIC field service requests must be completed and received by Litesource a minimum of 14 Business Days prior to requiring a lighting controls technician on site.


BASIC field service includes:

1 hour for a Pre-Installation Meeting held remotely via Microsoft Teams or in person at Litesource’s Raleigh office. The purpose of this meeting is to familiarize the Electrical Contractor with the lighting controls provided for the project. Topics include but are not limited to; submittal drawings review, product operation and design intent, installation requirements and common installation issues to avoid. The Electrical Contractor is responsible for scheduling this meeting with Litesource prior to lighting controls rough-in. The Electrical Contractor must have access to a physical full-sized, color submittal drawing or a .pdf of the drawing for this meeting.
Confirmation phone call within 24 hours of this request submission between the Lighting Controls Technician and the EC contact listed on this request form. This call is for coordinating the exact date, time of day and location of the required field service appointment and to answer any preliminary questions.
Basic functionality check for all areas at the start of the appointment to identify any issues and ensure site readiness.
Up to 1.5 hours assisting the Electrical Contractor troubleshoot electrical wiring and controls cabling.
On-Site programming of the lighting controls per the Sequence of Operation (SOO) provided in the Construction Documents or per current NC Energy Code requirements if a SOO is not provided or available.
Identification of any product manufacturing or firmware defects affecting the functionality of the system
Verification of any time schedules required.
DMX programming if DMX controls are provided by Litesource.
Coordination with Systems Integrator for BACnet Integration when specified.
2-hours On-Site Owner/End User Training
EC and Owner sign-off
A Field Service Report will be provided to the E.C. for record.

BASIC field service excludes:
Re-terminating bad cables
Sensor Tuning
Return Trips due to Site Readiness
Weekend / After Hours / Holiday work
Advanced

ADVANCED field service requests must be completed and received by Litesource a minimum of 14 Business Days prior to requiring a lighting controls technician on site.

ADVANCED field service includes all BASIC items and the items below:

1 qty 2-hour On-site Pre-Installation Meeting with a Litesource controls representative.
2 qty 2-hour On-Site Installation Checkup visits.
1 qty 2-hour On-Site Pre-Commissioning walk-through visit.
Up to 3 hours troubleshooting wiring or cabling issues.
Sensor Tuning and testing of all sensors.
Generating relay panel schedules if required.

ADVANCED field service excludes:
Re-terminating bad cables
Return Trips due to Site Readiness
Weekend / After Hours / Holiday work
A Purchase Order must be provided to Litesource before ADVANCED field service is scheduled.

Return Trip Service Work

(CHOOSE IF THE ORIGINAL FIELD SERVICE WAS UNABLE TO BE COMPLETED DUE TO SITE READINESS OR OWNER TRAINING NOT SCHEDULED)

RETURN TRIPS for field service requests must be completed and received by Litesource a minimum of 14 days prior to requiring a lighting controls technician on site.

A Purchase Order must be provided to Litesource before a Return Trip is scheduled.

Warranty Work

If selected, a quote will be provided for this service. If Litesource receives a lighting controls order from this opportunity, a credit for this service will be reflected in the order.

Controls System Evaluation/Replacement

Included if manufacturing defects, faulty devices or programming issues are found by the Lighting Controls Technician during Field Service.

Electrical Contractor Site Readiness Responsibilities

All control devices have been installed per the manufacturer’s installation instructions in locations per the approved submittal drawings and are powered. Any device damaged during installation has been replaced and is powered. No devices are missing our un-installed.
All Lighting Fixtures are installed and powered. If fixtures have not been installed in a specific zone due to backorder, long lead time or owner-furnished, a temporary fixture has been installed. Control functionality cannot be verified if no lighting fixture is connected to the associated control zone. A temporary fixture allows the technician the ability to check that the associated controller is operating correctly.
All 0-10V wiring has been checked for proper installation. 0-10V wiring can be checked without the use of controls by shorting the 0-10V wires together. Shorting the + and the – 0-10V wires will dim all fixtures in a control zone sharing the same 0-10V pair to the lowest level or off depending on the driver. Opening the wires will result in the fixtures being full output.
All switch legs have been checked and match up with the 0-10V dimming wires for the associated control zone.
All pre-terminated Category (CAT5e/CAT6) cables have been installed between the devices as shown on the submittal drawings. Cable routing can differ from the submittal drawings but not how the devices are shown interconnected. All cables have been plugged into the required devices.
Field-terminated Category cables have been provided by the installing contractor, they have been terminated per TIA/568 B, have been tested with a LAN tester and are seated securely in the devices.

SITE TERMINATED CATEGORY CABLES SHOULD BE AVOIDED DUE TO THE NUMBER OF IMPROPER TERMINATIONS FREQUENTLY FOUND AND SHOULD ONLY BE USED FOR LONG RUNS OVER 100’ OR WHEN A PROJECT REQUIRES CATEGORY CABLES BE INSTALLED IN CONDUIT.
UL924 devices have been wired per the manufacturer’s installation instructions and tested. Emergency circuits are energized, and emergency neutrals are NOT shared with normal neutrals. 0-10V dimming wires are connected such that emergency lighting and normal lighting in the same area dim correctly together.
All occupancy or vacancy sensors have been adjusted (“tuned”) beyond the factory defaults per the construction documents, specifications or owner’s requirements.

* All sensors are capable of being either Occupancy (auto on/auto off) or Vacancy (manual on/auto off). Electrical Contractor is responsible for ensuring all sensors and/or powerpacks are wired and adjusted for either Occupancy or Vacancy depending on what is specified on the construction documents.

* Tuning sensors is required to ensure the sensor operates effectively in the space and location it is installed. Sensitivity/Time Out trim pots, buttons or dip switches shall be adjusted per the manufacturer’s installation instructions. Lights that stay on, turn off prematurely or do not turn on at all are directly related to sensor tuning. Sensor tuning IS NOT included in BASIC field service. Litesource will not perform a site visit to troubleshoot sensor operation.
Relay Panel(s) installed and powered, all relay schedules have been typed or written describing each load and provided at each relay panel.
If the project requires a time schedule to control lighting, Electrical Contractor shall provide the Technician required On/Off times and days for each area where a time schedule is required.
Integration: If the project requires integration into another control system, the Electrical Contractor shall schedule the other control system vendors to be present when the Technician is on-site. Any cabling or wiring between the Lighting Control system and another control system shall be by the Electrical Contractor.
Site has will be prepped for Field Service. Preparation includes: For Digital Systems: Removal of ceiling tiles where room controllers are installed. Digital Switches are unscrewed from their wall boxes allowing access to connecting cables Ladders are available for the technician. As-Built with cable paths as installed are available If DMX fixtures are installed, the addressable DMX module is accessible without requiring the technician to disassemble the fixture. If BACnet integration is required, the Systems Integrator contact information has been provided to Litesource. Electrician responsible for the lighting controls installation is available during the Field Service.
NOTE TO THE ELECTRICAL CONTRACTOR:(Required)
Items 1-7 above are the most common issues seen during startup visits.

Unless ADVANCED Field Service has been chosen, ALL 0-10V, switch leg, incorrect Category Cable routing/ termination issues and improperly wired UL924 devices are the responsibility of the Electrical Contractor and not the Lighting Controls Technician or Litesource. The Lighting Controls Technician may provide on-site guidance to the Electrical Contractor for corrections to be made but it is the sole discretion of the Lighting Controls Technician to deem whether the site is ready for Field Service. Any additional trip required will be considered a Billable Return Trip.


Billable Return Trip

If the Lighting Controls Technician arrives on site to find the lighting controls and/or lighting fixture installation is incomplete, installation quality is poor/ not per manufacturer’s installations instructions and/or the site readiness has been misrepresented, the Field Service may not be completed as scheduled or may be canceled at the discretion of Litesource. In either case, if a Billable Return Trip becomes necessary, the Electrical Contractor is responsible for the cost of all additional trips. A quote for the Billable Return trip will be provided to the Electrical Distributor associated with the project’s original controls order. A Purchase Order must be provided to Litesource before scheduling the return trip. Leadtime for Return Trips are 14 days from receipt of the Purchase Order.


The Lighting Controls Technician will provide a punch list to the Electrical Contractor listing corrections that need to be made prior to the Return Trip.


If corrections can’t be made to provide complete and correct controls functionality within the dates assigned for the initial Field Service, the Electrical Contractor is responsible for rescheduling the Field Service as a Return Trip using the webform located here:

A quote will be provided to the customer by Litesource. The Return Trip Date will be scheduled after Litesource receives a Purchase Order.


Under no circumstances is Litesource responsible for back-charges related to construction schedule delays due to electrical installation issues which prevents the Lighting Controls Technician from completing programming and training within the timeframe established.
I understand my responsibilities, as an employee or representative of the Electrical Contractor listed in this Field Service Request, to ensure the site is ready or will be ready for Lighting Controls Field Service on the dates chosen.
I understand that if Lighting Controls Field Service including Owner Training cannot be completed in full at the time the Lighting Controls Technician is scheduled, that I am responsible for all costs associated with a Billable Return Trip.
